Claim Form Instructions

If you were a California resident from May 9, 2023, to and through July 11, 2024, and may have had your personally identifiable information or protected health information disclosed to third party entities, as a result of using the Defendant’s website, ChooseKetamine.com and/or ChooseYourHorizon.com, while located in California, then you may be eligible for benefits.

To receive benefits, you must complete a Claim Form on or before April 23, 2026, unless you received notice from the Settlement Administrator containing a Unique ID and PIN. If you received notice, you do NOT need to submit a Claim Form, and your Cash Award will be sent to you by check at the address identified on the Notice if the Settlement is approved.
